CALL TO ACTION- Patient feedback about the future of Harold Street

PCS took over Harold Street in July 2023. The ICB (who commission the service) wish to review the future of the practice. The survey aims to ensure that the public voice is heard and shapes the proposals to be developed, providing insight into the impact any changes may have on local people and patients. 

The engagement exercise will run from 2 December 2024 to 13 January 2025. All patients will be informed of the survey by their chosen method of communication (SMS or letter) and in addition posters and copies for the survey will be available in practice. Practice staff will sign-post patients to the survey when visiting the practice.

The survey will be available in the languages most frequently spoken by practice patients (Arabic, Somali and English).

You can post the survey back to the freepost address or drop it off in practice. Please make sure your voice is heard.
